What to Bring for Your First Consultation

When you come in for your appointment, please arrive about 15 minutes early. Here’s a quick checklist of what to bring:

  • Your insurance card and coverage details
  • A photo ID (like your driver’s license)
  • A signed medical record release form
  • Any co-payment or self-payment needed

Important Instructions for Semen Collection

If you’re collecting a semen sample, either at home or in our office, follow these steps to ensure accuracy and avoid unnecessary repeat testing:

  1. Shower and clean your genital area thoroughly before collection.
  2. Wear clean clothes to keep the sample free from bacteria.
  3. Use only the sterile collection cup provided by our lab—other containers might mess with the sperm quality.
  4. Refrain from ejaculation for at least 2 full days, but no longer than 5 days before collection.
  5. Avoid soaps, creams, or any lubricants unless they’re provided by your doctor or lab.
  6. If you’ve had a fever or exposure to high heat recently, let your referring doctor know before your appointment.

If you’re collecting from home, make sure to grab a collection cup from our lab beforehand. Your sample should arrive within an hour, and don’t forget to label the cup with your name and date of birth!
