Many fertility clinics typically schedule frozen embryo transfers (FET) on set days, using estrogen and progesterone to prepare the uterine lining for implantation. This method allows clinics to manage their schedules better, avoiding weekend procedures. However, patients must continue hormone therapy for up to 12 weeks to support the pregnancy.
Recently, more clinics have begun to offer natural cycle FETs, which are timed to the patient’s natural ovulation with minimal medication. This shift allows for more flexibility, enabling transfers any day of the week. Despite the benefits, some clinics are still hesitant to adopt this approach, mainly due to a lack of extensive studies comparing success rates between natural and programmed cycles. The few studies available have been limited in scope.
At our clinic, we have always advocated for natural cycle FETs. With approximately 400 FETs performed each year, we’ve amassed significant data for analysis. While most of the patients in our study underwent natural cycles, some required programmed cycles due to irregular ovulation or the need for precise scheduling, especially for those traveling long distances.
Our research analyzed 1,378 frozen embryo transfers conducted from 2000 to 2005. Among these, 934 involved patients using their own eggs, while 444 utilized donor eggs. The key takeaway? There was no noticeable difference in pregnancy rates between those who had transfers timed with natural ovulation and those who underwent hormone preparation.
Natural cycle FETs are often preferred by patients due to their lower costs and the lack of blood tests or frequent ultrasounds. Patients appreciate this less invasive option compared to the traditional programmed method, and we now have solid evidence to support our recommendation of natural cycle FETs. Preliminary findings from our study were recently presented at a conference and are currently under review for publication in a leading reproductive health journal.
